The USPS 1583 form, is a form that needs to be filled out by anyone seeking mail service at ETX Depot. This is a requirement set by the USPS, which regulates how mail is received & handled. It serves as an authorization for ETX Depot to process & handle mail on your behalf. This form is kept on file by the USPS so that they can track the identity of those who use the address. Please note, that the USPS 1583 Form does not change your address with the USPS or tells ETX Depot where to forward your mail.
3. Email ETX Depot at info@etxdepot.com to set up a time to drop off the Mail Service Agreement and USPS 1583 Form
The USPS 1583 Form needs to be notarized. ETX Depot offers notary services and can notarize the form at time of delivery. Please note, two forms of identification will be required at time of drop off.
Accepted forms of ID: Valid driver’s license, passport from the USA, US permanent record card, certificate of naturalization, University student ID, passport, lease/mortgage or deed of trust, voter or vehicle registration card, home or vehicle insurance policy, utility bill, form I-94 with a arrival & departure.
Non-accepted forms of ID: Social security cards, credit cards, corporate ID’s, birth certificates.
